That is almost always a waterproofing problem from the original install. It is also the reason we do not cut corners behind the wall — that is the part of the job that actually matters.

My floor feels soft near the tub. Is that a big problem?
It usually means water has been getting to the subfloor for a while. Caught now, it is a manageable repair we do all the time. Ignored, it gets expensive. Either way we will show you exactly what we find and what it costs to fix.
Can you make a bathroom easier to use for someone with mobility issues?
Yes. Walk-in showers, grab bars anchored into blocking (not just drywall), comfort-height toilets, and wider doorways. We do this work regularly and it does not have to look institutional.
